Just FYI, the "Long URL" box overruns the box on the right hand side on Linux Firefox. I've seen this before. The element will require a width: attribute; html size won't do what you want.
You are also generating two <html> and two <body> tags.
And you have a problem with closing tags, here:
Btw, it's worth avoiding html attributes and using 100% CSS.
